Why is transosseous arthroscopy considered the gold standard in rotator cuff repair?
The transosseous approach is generally recognized as the gold standard for surgical repair of the rotator cuff.
We can list here the main reasons:
- Widely recognized by the scientific community
- Numerous supporting publications
- No hardware in the footprint
- Possibility to decorticate
- Larger tendon-bone contact area
- Uniform force distribution - no load peaks
- Biologically better repair
Transosseous Arthroscopy: advantages
What are the advantages for the patient? And for the physician?
With the transosseous approach, you have the ability to manage any type of rotator cuff injury.
Combined with a proper rehabilitation protocol, patients have a faster functional recovery and the rehabilitation phase has less pain than other comparable procedures.
On the other hand, for the physician, transosseous repair means:
- Biologically better repair
- Optimal management of revision cases, cysts and osteoporotic bone (with augmentation)
- Cost-effective
Transosseous Arthroscopy: critical issues
But What are the main errors and critical issues that can be encountered?
As with other techniques, critical issues or steps that require a major learning curve may be encountered in TO.
We have identified the following:
- How to load sutures in a Transosseous Tunnel

- How to access the lateral part of the greater tuberosity: what are the difficulties when going distal, and can the axillary nerve be a problem?

L'artroscopia transossea è genericamente riconosciuto come il gold standard per la riparazione chirurgica della cuffia dei rotatori.
Possiamo elencare qui le principali motivazioni:
- Ampiamente riconosciuto dalla comunica scientifica
- Numerose pubblicazioni a sostegno
- No hardware nel foot-print
- Possibilità di decorticare
- Maggiore superfice di contatto tendine – osso
- Uniforme distribuzione della forza – no picchi di carico
- Riparazione biologicamente migliore
Artroscopia Transossea: vantaggi
Quali sono i vantaggi per il paziente? E per il medico?
Grazie all’approccio transosseo, si ha la possibilità di gestire qualunque tipo di lesione della cuffia dei rotatori.
Unito ad un protocollo corretto di riabilitazione, i pazienti hanno un recupero funzionale più rapido e la fase riabilitativa presenta meno dolore rispetto ad altre procedure comparabili.
Dall’altro lato, per il medico la riparazione transossea significa:
• Riparazione biologicamente migliore
• Gestione ottimale di casi di revisione, cisti e osso osteoporotico (con augmentation)
• Cost-effective

Ma quali sono i principali errori e le principali criticità che si possono incontrare?
Come per altre tecniche, anche nel transosseo si possono incontrare criticità o passaggi che richiedono una learning curve maggiore.
Abbiamo identificato le seguenti:
• Come caricare le suture in un Tunnel transosseo
• Come effettuare tunnel multipli
• Tunnel convergenti, come fare?
• Come accedere alla parte laterale della grande tuberosità: quali le difficoltà quando si va distali, e può il nervo ascellare essere un problema?
• Fare il nodo in una tecnica transossea è equivalente all’approccio con le ancore?
• Quali sono le insidie con le ossa osteoporotiche? Esiste un modo per superare il problema?
• Augmentation, come rimedio allo sfilamento delle suture?
